15、根據(jù)實(shí)際情況有所增減。6具體指標(biāo):由部門(單位)或受托中介機(jī)構(gòu)設(shè)置并商財政部門研究確定,設(shè)置時可參照“浙江省項(xiàng)目支出評價指標(biāo)一覽表”(附2)。7評價等次:按照綜合得分(S)的分值填寫相應(yīng)的評價等次。評價等次分為優(yōu)秀(S95)、良好(95S85)、合格(85S60)、不合格(S60)4個評價等次。8評價人員:填寫參與項(xiàng)目評價的所有成員名單,并由各成員簽字確認(rèn)。四、評價報告文字部分(報告綜述)評價機(jī)構(gòu)應(yīng)根據(jù)下列提綱對各項(xiàng)內(nèi)容進(jìn)行詳細(xì)說明:(一)項(xiàng)目基本概況1.項(xiàng)目立項(xiàng):項(xiàng)目的實(shí)施依據(jù)、涉及范圍、主要內(nèi)容、用途、績效目標(biāo)以及預(yù)期主要經(jīng)濟(jì)、社會、生態(tài)環(huán)境效益等。2.項(xiàng)目執(zhí)行:項(xiàng)目的進(jìn)展及目標(biāo)、計(jì)劃調(diào)整
16、情況;績效總體目標(biāo)和階段性績效目標(biāo)的完成情況;項(xiàng)目總投入,財政撥款、自籌資金落實(shí)、實(shí)際支出情況;主要經(jīng)濟(jì)、社會、環(huán)境效益及可持續(xù)影響等;財務(wù)管理情況;項(xiàng)目管理制度及執(zhí)行情況等。(二)項(xiàng)目績效及評價結(jié)論對項(xiàng)目實(shí)施后所取得的實(shí)際績效進(jìn)行認(rèn)真梳理與總結(jié),并根據(jù)評價得分給出相應(yīng)評價等次。(三)評價發(fā)現(xiàn)的問題對項(xiàng)目實(shí)施和資金使用過程中存在的有關(guān)問題進(jìn)行總結(jié)與歸納。(四)相關(guān)意見與建議對評價過程中發(fā)現(xiàn)的問題進(jìn)行認(rèn)真分析與說明,并針對有關(guān)問題提出相應(yīng)的整改意見與建議。(五)附件評價機(jī)構(gòu)認(rèn)為需要作為評價報告附件的有關(guān)文件、資料等,以進(jìn)一步解釋和證明報告所反映的相關(guān)內(nèi)容。五、其他要求評價報告統(tǒng)一使用A4紙打印、
