1、某某有限公司職業病危害因素告知書（勞動合同補充約定）某某有限公司 （以下簡稱甲方） 與 （以下簡稱乙方）依據 中華人民共和國職業病防治法 和有關法律、法規， 甲乙雙方經平等協商， 自愿簽訂本補充約定。ThisSupplementaryagreementoflaborcontractismadebetweenYourCompanyLtd(hereinafterreferredtoas“TheCompany)and(hereinafterreferredtoasthe“TheEmployee)inaccordancewithLawofOccupationalDiseasePreventionofP

2、RCandRelevantLegislation,subjecttomutualagreementandvoluntarysigned.一、甲方權利與義務1 .負責如實將乙方工作過程中可能產生職業病危害及其后果、防護措施、待遇告知乙方，填寫職業病危害因素告知表（見附表1）2 .負責安排乙方上崗前、崗中、離崗以及發生急性職業危害事故時不同時期的職業健康檢查，并將檢查結果如實告知乙方，檢查費用由甲方承擔。4 .負責對乙方進行上崗前和在崗期間職業安全衛生培訓， 指導乙方正確使用相關職業病防護設備和個人職業病防護用品。5 .按照國家規定，使用達標的生產工藝設備、技術、材料，并對乙方工作區域進行職業危害

3、防護， 且按規定周期進行職業危害日常監測以及年度檢測、評價，將結果在危害崗位中告知。ItemI:RightsandobligationsofTheCompany1. TheCompanyarrangesoccupationalhealthexaminationofpre-employment,on-job,pre-resignandhavesufferedacuteoccupationalpoisoninghazardsforEmployees.Thecompanyshallbearallthecostsandprovidetheexaminationresult.4. TheCompanyarrangesthepre-employmentandonjobtheoccupationalhealthandsecuritytrain

5、ing,instructTheEmployeeforthecorrectutilizationofoccupationaldiseasepreventionequipmentandindividualusedoccupationaldiseasepreventionarticles.5. Inaccordancewithstateregulations,TheCompanyuseofstandardproductionprocessequipment,technology,materials.Andimplementoccupationalhazardsprotectionforthework

6、areaofTheEmployee.Andinaccordancewithregulations,TheCompanyshallperiodicallyperformoccupationalhazardsofdailymonitoringandtheannualinspection,evaluation,andinformedresult.6.不得安排處在孕期、哺乳期的乙方從事對本人和胎兒、嬰兒有危害的作業。6 .按照工傷保險條例相關規定，對因甲方工作環境導致乙方患職業病的，進行救治、補貼。7 .在勞動合同存續期間，因乙方工作崗位、內容變更致使接觸的職業病危害因素改變時，甲方應向乙方告知，并與

7、乙方重新簽訂職業危害變更告知表（見附表 2）二、乙方權利與義務1 .自覺遵守甲方制定的崗位職業衛生操作規程和制度。2 .正確使用職業病防護設備和個人職業病防護用品。3 .積極參加職業衛生培訓。4 .定期參加職業病健康體檢5 .發現職業病危害隱患事故應及時報告甲方6 .樹立自我保護意識，積極配合甲方，避免職業病發生。7 .離崗時，應按照規定參加甲方組織的離崗職業健康檢查。5.TheCompanydonotassigningoperationharmfultothehealthoftheembryosorbabiesforFemaleworkersduringtheirpregnantandlac

9、ange,TheCompanyhavetoshallinformTheEmployee,andre-signedtheoccupationalhazardschangeform(AttachmentII)ItemII:RightsandobligationsofTheEmployeeItemI:TheEmployeemustbeobeyTheCompanyestablishtheoccupationalhealthmanagementsystemandoperatingrules.ItemII:TheEmployeemustbeproperuseofpersonaloccupationaldi

10、seasesprotectiveequipmentandoccupationaldiseasesprotectiveequipment.ItemIII:TheEmployeemustbeactivelyparticipateinoccupationalhealthtraining.ItemIV:TheEmployeeshouldregularlyparticipateinoccupationalhealthcheckItemV:TheEmployeeshouldinformTheCompanyforfoundtherisksandtheaccidentofoccupationalhazards

11、.ItemVI:TheEmployeemustestablishcertainawarenessofself-protection,activelycooperatewithTheCompany,topreventoccupationaldiseases.ItemVII:WhenTheEmployeeleavecompany,shouldbeparticipatingTheCompanyorganizedoccupationalhealthchecks.如甲方未按照規定如實向乙方告知，乙方有權拒ItemIII:IfTheCompanyfailingtotruthfullyinform絕從事職業
