CCCF-ZH-013 2008年8月1日 第六次修訂 第1頁 共14頁消防產品型式認可申請書APPLICATION FOR TYPE-APPROVAL OF FIRE PRODUCTS首次申請/First Application 擴大范圍申請/Scope Expansion 復評換證申請/Renewal換版申請/Format Change 申請企業(yè)名稱/ Application product category:(加蓋公章/Seal)生產廠名稱/ Factory:申請產品類別/Category of Product:填報日期/Submission Date (yyyy/mm/dd)： 年 月 日公安部消防產品合格評定中心印制Published by China Certification Center for Fire Products, Ministry of Public Security, PRC.CCCF-ZH-0132008年8月1日 第六次修訂 第14頁 共14頁申 請 企 業(yè) 須 知1、申請企業(yè)必須嚴格按照消防類產品型式認可實施規(guī)則中工廠基本條件的要求建立并實施有效的質量管理，確保企業(yè)的質量保證能力符合實施規(guī)則的基本要求；2、申請企業(yè)必須符合消防類產品型式認可實施規(guī)則中生產設備及檢驗設備的最低配置要求；3、申請企業(yè)須嚴格遵守型式認可程序及時限要求，正常情況下，接到工廠檢查通知后的30日內應接受工廠條件檢查，并應在規(guī)定時間內完成對不符合項的整改工作，自抽樣之日起15日內將樣品送到指定檢測機構進行檢驗，并按期繳納相關費用；如遇特殊原因，必須及時通報公安部消防產品合格評定中心；4、申請企業(yè)獲證后，必須嚴格按照消防類產品型式認可實施規(guī)則的規(guī)定，正確使用型式認可證書和標志。對有違規(guī)者，公安部消防產品合格評定中心將進行嚴肅處理。5、申請企業(yè)獲證后，所生產及銷售的產品必須與發(fā)證檢驗樣品保持一致，產品質量必須符合型式認可規(guī)則和相關標準要求。若必須對涉及產品一致性的關鍵因素，如關鍵原器件、原材料、關鍵設計及主要生產工藝發(fā)生變更，以及生產場地搬遷、法人及主要技術人員發(fā)生變更時，獲證企業(yè)必須及時向公安部消防產品合格評定中心申報，由中心按照消防類產品型式認可實施規(guī)則要求安排確認工作。未經中心確認，不得生產及銷售產品，對違規(guī)者，公安部消防產品合格評定中心將進行嚴肅處理，由此而導致的一切責任均由違規(guī)企業(yè)承擔。The Notice of the Applicant1. The Applicant must not only constitute a quality control and bring into effect which according to the request of the factorys postulate, but also make sure the quality guarantee capacity of the corporation according to the request of the implement rules.2. The applicant must measure up the lowest configuration requirement of producing equipment and checking equipment that refers to the The Implement Rules Of The fire fighting product Type-Approval.3. The applicant must follow the authentication procedure and the time limit requirement. In normal condition, the enterprise should accept the work condition inspection within 30 days after receive the information, and finish the modification of the unaccepted item within stated time. The enterprise should send the sample to the appointed examine institution in 15days after the inspection, and pay for the related bill. The enterprise should declare to CCCF in time if there is any special case.4. After getting the certificate, the applicant must follow the implement rules strictly and use authorized certificate and symbol correctly. CCCF will sternly punish the foul.5. After getting the certificate, the applicant must keep the products they produce and sell the same with the inspecting samples. The quality of the products must measure up the authentication regulations and the correlative standards. If the some key factors that related to the coherence of the products have to be modified, such as key source apparatus, raw material, core design and primary produce technique, as well as the workplace, juridical person and key technical personnel change, the enterprise must declare to CCCF. And CCCF validates the situation according to the implement rules. The enterprise cant produce and sell any products without the validation. CCCF will sternly punish the foul. And the foul is charged of all the correlative obligation.聲 明我代表申請企業(yè)鄭重聲明：1本企業(yè)遵循“誠信第一”的基本原則，承諾遵守國家有關法律法規(guī)及公安部消防產品合格評定中心（以下簡稱CCCF）制定的型式認可實施規(guī)則的所有要求及型式認可程序。2本企業(yè)同意按型式認可規(guī)則的要求提供評價所需文件和資料，所提供的信息均真實有效，否則承擔由此引起的一切法律責任。3本企業(yè)保證按型式認可合同及相關要求支付型式認可費用，并為實施型式認可產品型式認可工作提供所需要的資源。4、本企業(yè)將妥善保管檢驗報告、工廠檢查報告和型式認可變更確認等型式認可的相關資料，以備監(jiān)督檢查、飛行檢查及有關證后監(jiān)督工作時使用，公安部消防產品合格評定中心將不承擔獲得產品合格型式認可的生產廠或銷售商應承擔的法律責任。5、本企業(yè)完全接受申請企業(yè)須知中所涉及的全部內容。StatementI, on the behalf of the Applicant, make the statement hereby:1.
