工地所在的當(dāng)?shù)刂鞴墚?dāng)局和工業(yè)部門所規(guī)定的有關(guān)安全、衛(wèi)生和勞資關(guān)系的一切要求和慣例。ThehandbookasktheContractorwillhavefullyunderstandandobserveallthesafetyrequirementandpracticeonsafety,healthandlaborspecifiedbythelocalauthoritiesandtheindustrydepartment.工作場地的使用條件Conditionofsite工作人員Personnel承包商的雇員詳情Contractor’spersonnel管理人員Managerialpersonnel承包商名稱和承包合同編號NameofContractorandtheNo.oftheContract管理人員的姓名Namelistofthemanagerialpersonnel管理人員在工地上所擔(dān)任的職位Positionsofthemanagerialpersonnel管理人員在工地上開始工作的日期Dateofenteringsiteofthemanagerialpersonnel工人Labors工程師可以在工地開工的三個工作日以前要求承包商提供下列的工人詳情，這項要求可在接收工地時通知承包商TheengineerwillasktheContractortoprovidelistoflaborsbelow3daysbeforecommencementwhenhereceivesthesite.工人的姓名和類別Nameandtypeofworkoflabors操作機(jī)器設(shè)備或是作為裝配工工類的工作能力證明書Operationcertificateformechanicalworkersorfitters.衛(wèi)生與安全的執(zhí)行須知Noticesofsafetyandhealth承包商的一切雇員必須在工地開工以前進(jìn)行<<衛(wèi)生與安全的執(zhí)行須知>>培訓(xùn)班其教學(xué)內(nèi)容應(yīng)適合一切雇員的工種和工作環(huán)境。AlltheemployeeoftheContractorwillbetrainedwith“Noticesofsafetyandhealth”accordingtothetypeofworkandenvironmentoftheemployees.人事檔案的保存Keepingofpersonalfiles承包商必須保存一份最新的人事檔案，列明在工地上工作的全部人員。TheContractorwillkeeplatestpersonalfileonwhichallthepersonnelarelisted.工地設(shè)施和各項服務(wù)Sitefacilitiesandallservices總則General承包商必須為了完成承包工程提供一切所需的工地設(shè)施和各項服務(wù)，承包商所提供的工地設(shè)施只準(zhǔn)許設(shè)置在經(jīng)工程師批準(zhǔn)的位置上，一但承包工程竣工，承包商所提供的一切工地設(shè)施，包括水、電服務(wù)線路，均須由承包商負(fù)責(zé)拆除，并恢復(fù)廠區(qū)面貌，以滿足業(yè)主的要求。TheContractorwillprovidesitetemporaryfacilitiesandalltheservicestocarryouttheproject.ThefacilitiesonlycanbesetuponthepositionapprovedbyEngineer.TheContactorwillremoveallthetemporaryfacilitiesincludingwaterandelectriclinestoresumethefactoryenvironmenttomeettheOwner’srequirement.工地辦公室Siteoffice承包商必須提供自用的工地辦公室，以有效地管理其承包工程。TheContractorwillbuildhissiteofficetocontrol大臨設(shè)施Temporaryfacilities承包商可以根據(jù)平面布置設(shè)計、在廠區(qū)內(nèi)搭建必要的施工用臨時設(shè)施。TheContractorwillbuildtemporaryfacilitiesinthefactoryareainaccordancewiththeplanearrangementdesign.廁所W.C.可以要求承包商提供臨時結(jié)構(gòu)的廁所廁所必須隨時保持清潔、衛(wèi)生狀態(tài)TheContractormaybeaskedtoprovideW.C.inthetemporaryfacilities.TheW.C.willbekeptcleanandsanitationstate.供水Watersupply承包商必須負(fù)責(zé)安裝工地所需的供水臨時管道和其它附屬裝置當(dāng)承包工程需要大量供水時（例如：土石方工程、現(xiàn)場焊裝的試驗等等），承包商必須跟工程師一起作出具體安排來滿足這些需要。TheContractorwillbeinchargeofinstallationofthewatersupplypipeandtheauxiliarydevicesforthetemporaryfacilities.電話Telephone工地辦公室必須配備電話以供市內(nèi)通話。TelephonewillbeprovidedinthesiteofficeforcommunicationinShanghai.急救Firstaid承包商必須提供一個急救站TheContractorwillprovideafirstaidsatiation.一般規(guī)章制度Generalrules承包商及其雇員們務(wù)須遵守下列各項工地的規(guī)章制度，凡不滿十八足歲者（經(jīng)批準(zhǔn)的學(xué)徒除外）不準(zhǔn)進(jìn)入操作工地.酒類或藥物（醫(yī)生處方的藥物除外）不準(zhǔn)帶入工地，受酒類或藥物影響的人員不準(zhǔn)進(jìn)入工地。槍支不準(zhǔn)帶入工地工地內(nèi)不準(zhǔn)賭博、胡鬧和打架；否則會導(dǎo)致被開除出工地。ThefollowingruleswillbeobservedbytheContractor’semployees:Personunder18year-age(excludingapprovedapprentice)isnotallowedtoenterthesite.Alcoholicdrinksormedicine(excludingprescriptiondrug)isnotallowedbringingtosite.Personsaffectedbyalcoholicordrugisnotallowedtoentersite.Gunsarenotallowedtobringingtosite.Nogambling,beingmischievousandfight,orhewillbekickedoutfromthesite.5勞資糾紛Labor-capitaldispute總則General承包商必須將所發(fā)生的任何勞資糾紛的情況立即向工程師做出匯報TheContractorwillreportanylabor-capitaldisputehappenedtoEngineeratonce.罷工報告Strikereporting在勞資糾紛期間，承包商須將每次發(fā)生罷工或停工的人工損失詳情向呈報。TheContractorwillreportthedetailedsituationofeachstrikeinthelabor-capitaldispute.6承包合同的實施管理ManagementoftheConstructionContract會議和報告Meetingandreport除非工程師在特殊情況下放棄要求，承包商必須組織和/或出席下列各種會議，并須提供下列各項報告。TheContractorwillorganizeand/orattendthefollowingmeetingsandprovidethereportsunlesstheengineergivesupinspecialsituation.每日出勤人員周報Weeklyreportofdailyrollbook這份每周呈報一次的報告表示承包合同的各類雇員在工地上每日工作人數(shù)，包括所有的管理人員和工人。Thereportshowsthenumberofpersonnelincludingmanagerialpersonnelandworkersofallsortspresentedonsiteinlastweek.事故/事件報告Accident/incidentreport這份根據(jù)工地情況呈報的報告，提供有關(guān)承包商的雇員們的一切意外事故和小事件。 Thereportaccordingtothesitesituation,providealltheaccidentandsmallincidentoftheContractor’semployees.安全違反報告Reportofsituationagainstsafetyregulations承包商必須遵守一切安全條例，若承包商或其雇員未能照辦，工程師的安全員會向工程師辦公室發(fā)出一份安全違反報告，有關(guān)的承包商/雇員將會受到處分。TheContractorwillobserveallthesafetyregulations,iftheContractororhisemployeewillnotfollow,theengineer’ssafetymanwillsubmitareportofsituationagainstsafetyregulation,andtheContractor/hisemployeewillbepunished.7安全Safety承包商的責(zé)任TheContractor’sliabilities監(jiān)理組制訂的安全標(biāo)準(zhǔn)和安全程序的概要載列在本手冊“明文規(guī)定的工作程序”一節(jié)和“工程地區(qū)的安全”一節(jié)內(nèi)，這些有關(guān)安全的方針和程序，有些是專門適用于工地的，其余均適用于一切場合，承包商必須遵守本手冊所概述的一切適用的法令和工地的安全要求。Thesafetystandardandsafetyprocedureisincludedinthesection“DocumentSpecifiedProcedure”andthe“SafetyinConstructionArea”ofthishandbook,whichissuitableforsiteuse,theothersaresuitableforallcircumstance.TheContractorsmustfollowalltheadoptedlawsandsiteregulations.人身保護(hù)用具Personalprotectiveequipment承包商必須至少保證向工地上工作的一切人員提供下列穿戴的人身保護(hù)用具：安全帽、安全鞋工作安全所需的其它專門用具也須由承包商提供，例如手套、護(hù)目鏡等。TheContractorwillguaranteeatleastthefollowingpersonalprotectiveequipmentfortheallthepersonnelonsite:safetyhelmet,safetyshoeandotherspecialappliancessuchasgroove,gogglesetc.承包商的安全、衛(wèi)生與環(huán)境保護(hù)的組織結(jié)構(gòu)Contractor’sorganizationforsafety,healthandenvironment.承包商必須設(shè)置一個安全、衛(wèi)生與環(huán)境保護(hù)機(jī)構(gòu)，工程師的衛(wèi)生安全代表和承包商的衛(wèi)生、安全管理員共同督促工地的安全管理。承包商必須每周主持一次作會議記錄的安全、衛(wèi)生會議。工地安全、衛(wèi)生與環(huán)境保護(hù)的管理機(jī)構(gòu)在工地上應(yīng)設(shè)置下列的管理機(jī)構(gòu)TheContractorwillsetupanorganizationofsafety,healthandenvironment.Theengineer’sHSErepresentative,theContractor’sHSEmanwillsupervisesafetymanagementonsite.TheContractorwillheldweeklymeetingofHSEandkeeprecord.Thesitesafety,healthandenvironmentorganizationwillsetupfollowingmanagementorganization:工地的衛(wèi)生、安全執(zhí)行委員會：SiteExecutiveCommitteeofHSE:業(yè)主的安全代表TheEmployer’sHSErepresentative各承包商項目經(jīng)理Projectmanagersofallcontractors工程師衛(wèi)生安全管理員Theengineer’sHSErepresentative承包商的衛(wèi)生安全執(zhí)行委員會：TheContractor’sExecutiveCommitteeofHSE:承包商項目經(jīng)理Projectmanagerofallcontractors承包商的衛(wèi)生安全管理員theContractor’sHSErepresentative雇員選出的衛(wèi)生安全代表theemployee’sHSErepresentative承包商的衛(wèi)生、安全執(zhí)行委員會TheContractor’sExecutiveCommitteeofHSE:在工地開工的一周內(nèi)，承包商必須設(shè)置一個衛(wèi)生、安全執(zhí)行委員會在承包商的工程地區(qū)內(nèi)協(xié)助安全方面的管理工作。該執(zhí)委會的任務(wù)乃是在承包商的工程地區(qū)內(nèi)考慮一切安全事宜以及解決一切有關(guān)承包工程的安全問題。TheContractorwillestablishaHSEcommitteetoassistmanagementofHSEwithinoneweekaftercommencement.ThetaskoftheContractor’sHSEcommitteeistoconsiderandsolveallthesafetyaffairinthehisconstructionarea. 安全問題的解決程序Solvingproceduresofsafetyproblems本程序旨在盡可能不影響工程進(jìn)展的同時，來解決安全問題。本程序絕不減少各承包單位及其雇員們對于安全的義務(wù)和職責(zé)。Theprocedureintendstosolvesafetyproblemstoreducethebadinfluenceontheprogressaslessaspossible.TheprocedurewillneverreducetheobligationsandliabilitiesoftheContractorsandtheiremployees. 施工單位應(yīng)當(dāng)向作業(yè)人員提供安全防護(hù)用具和安全防護(hù)服裝，并書面告知危險崗位的操作規(guī)程和違章作業(yè)的危害。Contractorwillprovideoperatorsafetyprotectionappliancesandsafetycloth,andinformtheminwritingtheoperationproceduresofthedangerouspostandtheharmofoperatingagainstthesafetyrules. 作業(yè)人員有權(quán)對施工現(xiàn)場的作業(yè)條件、作業(yè)程序和作業(yè)方式中存在的安全問題提出批評、檢舉和控告，有權(quán)拒絕違章指揮和強(qiáng)令冒險作業(yè)。
