2、e on the site Permit to Work system.本程序的目的在于就現場作業的許可證制度提出指導性意見。1.2 The site handles a large diversity of work, so the potential for serious accidents is high. To prevent them it is vital that there should be safe systems of work. They will often be applied and enforced by means of a formal Permit to

3、 Work system.施工現場充滿了高強度的作業，因此發生重大事故的可能性也比較高。為了防患于未然，必須制定一套安全作業系統。其中用得比較多的是一種正式制度是作業許可證制度。1.3 All Critical Operations will be under the control of a Permit to Work system operated by the Site. Specific training will be given to all employees on the PTW system .All permit requests must be made at leas

4、t 24 hours prior to work commencing.所有關鍵作業行為均必須處于現場作業許可證制度的控制之下。全體員工均必須接受有關作業許可證制度的具體培訓。各種作業許可證都必須在作業開始之前至少提前二十四小時提出申請。1.4 Routine Operations will not be covered by Permit to Work.常規作業不需要辦理作業許可證。2. 5、e hot works in vicinity of the area that have inflammable material storage, enter into vessels/equipment or any confine space work, temporary electrical work electrical work, excavations deep than 300mm; use of radioactive, spray painting and grit blasting ,use of cartridge tools ,use of man baskets

6、, erection or removal of flooring and handrails on open steelwork, diving operations and explosive material,; pressure testing, lifting operations over 50 tons and multi-crane lifting work and any other operations which might be identified during the progress of the project that need to be covered u

7、nder the Permit to Work system. 關鍵作業-關鍵作業包括在材料存放有可燃性材料的區域附近進行的高溫(動火)作業、進入容器、設備和密閉空間作業、臨時性電氣作業、超過0.3米的開掘作業、放射性、噴漆和噴砂作業、使用沖擊工具、使用載人吊艙，載開放式鋼架上架設或移除地板和欄桿、潛水作業和易爆炸材料的使用、壓力測試作業、使用多臺(含兩臺)吊機起吊或超過五十噸以上的起吊作業以及項目進行過程中識別出來的其它適用作業許可證制度的各類作業。Routine Operations Operations in areas solely designated for operations

8、will not require Permit to Work even though critical operations are carried out. For example a workshop or grit blasting area where hot work is considered “normal operation” will not require Permit to Work. However these activities must be covered under other SSPs or special procedure for the area.

12、3.4 The duties of the safety engineer are:安全工程師的職責是：3.4.1 To be the co-coordinator of all permit work at the facility, including those permits which are suspended, but where the work is not complete充當本區域所有與作業許可證制度有關事宜的協調員，包括那些許可暫時收回，但工作尚未完成的作業的后續處理事宜。3.4.2 To keep the Construction Manager informed o

13、f any potentially hazardous situations arising from permit work, advise the Construction Manager of any activities that need to be bought under the control of the PTW system and to ensure all staff involved have an overview of significant ongoing work為了保證施工經理及時掌握由于作業而引起的各種潛在危險情況，應該加強與施工經理的溝通，及時提醒其開展

PROCEDURE程序4.1 General概述A Permit to Work system is a formal written system used to control certain types of work, which are ide

17、ntified as potentially hazardous. Essential features of a Permit to Work system are:作業許可證制度是一種對某些類型的作業進行控制的正式書面制度。這類作業一般是指那些具有潛在危險的活動。作業許可證制度的基本特征為：a. Clear identification of who may authorize particular jobs,明確地規定誰有權批準某些特殊的作業，b. Training and instruction in the issue and use of permits.就作業許可證的頒發和使用進

18、行必要的培訓和指導。c. Monitoring and auditing to ensure that the system works as intended. 對作業許可證的使用進行監督和審核，確保制度的實施滿足預期的目標.d. A Permit to work system is applied to protect personnel and plant. It consists of an organized and predefined HSE procedure forming a clear record of all foreseeable hazards that have

21、正確的防護服和設備 - Appropriate management are aware of the work in progress管理層知道工作正在進行中4.2 Permit Display許可證上的顯示內容4.2.1 Copies of a Permit to Work should be clearly displayed:應該在下列場合清楚地顯示作業許可證的內容：a) At the work site.在作業場所。b) In the permit co-ordination room, with additional copies at the worksite.在許可證辦公室，留

22、有作業現場許可證的副本。4.3 Permit Suspension許可暫時中止4.3.1 If for any reason work controlled by a permit has to be suspended then the permit will immediately be returned to the Superintendent who will acknowledge the fact and co-ordinate the return of all permits to the Permit Office.如果由于各種原因，原來受許可證控制的某項作業不得不暫時停止

24、ons.作業現場的情況發生變化4.3.3 After suspension it will be necessary, once permission has been given to recommence work, to have the permit re-validated before the work can restart.在許可證暫時收回以后，一旦允許重新開始作業，則在重新開始作業之前，必須對原頒發的許可證重新核準。4.4 Cross Reference交叉確認4.4.1 It is important to make sure that one activity under

25、 a Permit to Work does not create danger for another ongoing activity.在根據作業許可證進行某項作業時，必須確保不對其它正在進行中的工作構成威脅。這一點是非常重要的。4.4.2 The safety engineer should be aware of potential interaction and should check when a permit is prepared whether it impinges on one already issued.安全工程師應該清楚地意識到各種作業之間可能存在的關聯關系，在準

26、備簽發許可證時，應該仔細確認它是否與此前頒發的許可作業之間存在著沖突。4.5 Permit Revalidation許可證的重新核準4.5.1 When a permit is authorized by safety engineer , it will have a time expiry placed on it. This may vary form a few hours to a full shift or even several days, depending upon the nature of the work being undertaken and the freque

27、ncy at which the activity is repeated.安全工程師在簽發許可證時，應該注明許可證的有效時間。有效期有可能是幾個小時，也有可能是整個班次甚至是幾天，具體要由作業的性質以及作業重復的頻率決定。4.5.2 If, at the end of this time period, the work being controlled by the permit is not complete the permit may be revalidated by the responsible person, who shall ensure, together with t

30、ties under Permit to Work System 作業許可證制度所規定的責任4.7.1 Duties are placed on various persons with regard to the operation of a Permit to Work system. In general these are as follows:在執行作業許可證制度過程中，不同的人承擔著不同的責任。總的分工如下：a) The Contractor - who must ensure that all persons within his employment have a clear

31、understanding of how the PTW system operates. In the case of Contractors this is normally undertaken in conjunction with the Superintendent,承包商-負責保證他所雇傭的所有員工都能正確地理解作業許可證制度的運行方式。對承包商而言，他可能更多的是與現場主管合作執行這一制度。b) Each project should issue and co-ordinate the permits and identify those activities for whic
